Our Occupational Therapists use a range of assessments to determine goals and functional capacity, ensuring participants have access to the right level of support and NDIS funding.

Home and Living Assessments

Participants may be eligible for home and living supports if they have complex housing and support needs. Our Occupational Therapists assess eligibility for these supports and provide recommendations tailored to each participant’s needs. Our comprehensive home and living reports offer robust evidence to complement and strengthen the application process.

Our home and living assessments can include:

  • Independent Living Options (ILO).

Assistive Technology (AT)

Our Occupational Therapists conduct thorough assistive technology assessments to understand the unique needs of our participants, aiming to enhance occupational engagement and independence. We recognise that each participant requires tailored assistive technology solutions, so our team ensures a comprehensive trial and prescription process to find the optimal solution. We maintain strong relationships with local assistive technology providers to streamline the prescription process.

At PHW, our team has extensive experience prescribing a range of assistive technologies, including:

  • Low-cost and high-risk items such as shower chairs and four-wheeled walkers.
  • Mid-cost items like electric lift recliner chairs, adjustable hi/lo beds and mattresses, and mobility scooters.
  • High-cost items such as custom-scripted manual and power wheelchairs.

Home Modifications

Our team specialises in assessment and intervention for both minor and complex home modifications. We equip our Occupational Therapists to collaborate closely with participants to identify their specific home modification requirements, aiming to enhance home safety and promote independence in activities of daily living (ADLs).
